Position Summary

Located in Hervey Bay in sunny Queensland, St Stephens Hospital’s brand new facility provides the only not-for-profit private hospital service on the Fraser Coast.  As Australia’s first fully integrated digital hospital, this state-of-the-art ‘hospital of the future’ provides vital services and showcases how technology can transform the healthcare experience

 

Due to an increased demand for Allied Health services, we are pleased to be seeking a Discharge Planner to join our team in a new permanent part time opportunity.


Your Role:


You will provide high quality clinical services to patients, families and care givers in order to optimise psychosocial outcomes and achieve maximum benefits from health care services. 

 

As part of Allied Health, you’ll work collaboratively as part of a multi-disciplinary team, ensuring excellence in the provision of services and ensuring that all relevant services are provided in a safe, appropriate, efficient, effective and responsive environment.You will balance accountabilities of clinical practice, clinical governance, communication and workload management, whilst demonstrating our core values and a commitment to person-centred care. 

 

As a permanent part time opportunity, this role will be rostered two days a week.


Your Background:


As an experienced clinician, you will present with a minimum of 4 years nursing experience, demonstrating a high level of interpersonal and written communication skills and interdisciplinary teamwork.

You’ll demonstrate:

  • Bachelor of Nursing or equivalent with current AHPRA registration
  • Demonstrated ability to deliver a higher level of clinical reasoning and decision making that is based on current best practice.
  • Demonstrated organisational skills and ability to manage a more complex and variable caseload.
  • Demonstrated evidence of involvement in quality improvement or service delivery improvement initiatives.
  • Demonstrated commitment to continuing professional development.

As a Level 2 role, we offer $44.14 - $47.29 p/hour + Super commensurate with your experience. As a not-for-profit hospital, we also offer access to competitive salary-packaging options. Free parking is available on site.
